TRANSITION INFORMATION SHEET
Make a smooth transition from high school to college by enrolling with Disabled Students Programs and Services. We offer students a broad range of special services designed to support academic success. Through DSPS, high school students with accommodation needs can complete placement testing, receive assistance with class scheduling and arrange for support services.
To apply for DSPS services, follow the steps in the checklist below.
Complete the following:
1. _____ Create an e-mail account if you do not have one. For example, go to: www.gmail.com or www.hotmail.com
2. _____ Submit a Santiago Canyon College application for fall 2013. See the online application packet (PDF) for instructions on how to complete an application. Be sure to write down your login and password for CCCApply and keep it in a safe place. You will receive an email confirmation that your application has been successfully submitted.
Obtain high school records
3. _____ Most recent Psycho-Educational Assessment Team Report with latest cognitive and achievement test scores
4. _____ Last IEP or 504 plan
5. _____ Unofficial high school transcript
Make an appointment
6. _____ After all the above is completed, you (the student) must phone us at (714) 628-4860 to schedule an appointment for placement testing. Please be sure to have your SCC student ID number ready when you call.
Meet with DSPS Instructor
7. _____ Schedule an
appointment to meet with a DSPS instructor for fall class planning and program
registration.
